Bicon Integrated Abutment Crown™: Maxillary Left First Bicuspid
This case demonstrates the less than ideal uncovering of a two-stage implant, an implant level impression, shade recording, temporization, insertion and occlusal adjustment of a Bicon Integrated Abutment Crown™.

1. Occlusal view of implant well after the uncovering of the implant in a two-stage implant placement technique.

2. Guide pin inserted into implant well. An elliptical crestal incision with palatal reflection would have been more aesthetically desirable than the two vertical buccal incisions.

3. Impression post being inserted into the 3.0mm implant well.

4. Impression material being injected around the tapped and fully seated impression post.

5. Impression post being removed from implant well. If it were to have come out with the impression, it would indicate that it was not fully seated into the implant well.

6. A 3.0mm impression post being inserted into its corresponding implant analog.

7. An assembled impression post and implant analog.

8. The assembled post and analog being inserted into the impression.

9. Assembled post and analog in impression prior to the pouring of a soft tissue model.

10. Digital photograph being taken of B2 shade guide with a black background for e-mail transmission to the technician.

11. A 5.0mm x 4.0mm shouldered abutment being inserted into the implant with only finger pressure for the support of an interim crown restoration.

12. A wide one-piece acrylic sleeve being inserted onto the shouldered abutment.

13. One-piece acrylic sleeve snapped onto shouldered abutment.

14. Temporary composite material, “Integrity” by Dentsply Caulk, being injected onto one-piece acrylic sleeve.

15. Vacuum formed template being used to form the interim crown.

16. Gut suture being used to hold less than ideally designed buccal flap.

17. Finished Integrated Abutment Crown™ on soft-tissue model.

18. Finished Integrated Abutment Crown™ on stone model revealing white implant analog.

19. Mirror view of Integrated Abutment Crown™.

20. Interim crown revealing extent of gingival recession most likely due to the design of the flap.

21. Integrated Abutment Crown™ being inserted into the 3.0mm implant well.

22. Tapping on blue acrylic incisal orientation jig for initial seating of Integrated Abutment Crown™. After initial adjustments, it will be necessary to tap only on the Integrated Abutment Crown™ to achieve final seating.

23. Blue ink mark indicating need for adjustment.

24. Mesial palatal cusp being polished with a #7404 carbide bur.

25. Integrated Abutment Crown™ being polished with a nylon brush.

26. Integrated Abutment Crown™ being polished with soft white bristle brush and paste.

27. Completed Integrated Abutment Crown™ restoration revealing gingival aesthetics despite the scarring of the less than ideally designed buccal flap.

28. Radiograph of completed Integrated Abutment Crown™ restoration.
