Porcelain Fused to Metal: Three Unit Fixed Maxillary Anterior Bridge with an Unaesthetic Cervical Margin
This case demonstrates the clinical and laboratory technique for the retrofitting of an unsupported porcelain margin on a completed porcelain fused to metal fixed prosthesis.

     


1. Initial insertion of thirteen units of porcelain fused to metal restorations revealing an unacceptable showing of the titanium abutment of the maxillary right central incisor.


2. Closer view of the unacceptable display of metal at the cervical margin.


3. Gingival height being marked on abutment prior to its removal for final preparation in the laboratory.


4. View of bridge abutments prior to the removal of the central incisor abutment for final preparation in the laboratory.


5. Abutment seated in the prosthetic crown prior to its final preparation.


6. Separating liquid being applied to the prepared abutment.


7. Abutment prepared with a cervically adjusted buccal margin being inserted into the prosthetic crown.


8. Porcelain being added to the buccal cervical margin of the porcelain fused to metal prosthetic crown.


9. Porcelain being applied.


10. Additional porcelain being adapted.


11. Prosthesis after being removed from abutment.


12. Prosthesis with unsupported porcelain added to the labial margin of central incisor crown.


13. Prosthesis prior to its being inserted into the porcelain oven.

15. Orientation of prosthesis being confirmed with the use of a blue acrylic occlusal template or jig.


16. Extra-orally cemented left central incisor being tapped into the implant well in the long axis of its abutment shaft.


17. Prepared marginal surface of abutment being sand blasted with 50 micron Alumina.


18. Metalic coupler being applied to marginal area of abutment.


19. Ceramic coupler being applied to unsupported porcelain of the labial margin.


20. Bonding agent being applied to abutment.


21. Extra-oral cementing of maxillary central incisor abutment.


22. Removal of extraneous cement with cotton.
