Telescopic Restoration: Five Unit Polyceramic to Metal Mandibular Telescopic Bridge
This case demonstrates the versatility of restoring two non-parallel implants with a telescopic polyceramic to metal bridge.

1. Preoperative radiograph of failed porcelain to metal fixed prosthesis.

2. View of failed endodontically treated teeth.

3. Guide pin inserted into the 3.0mm implant well after its uncovering as part of a two stage surgical placement.

4. White 3.0mm impression post being inserted into the uncovered implant.

5. White impression post receiving the mandatory seating tap.

6. Anterior impression post being tapped into the implant well.

7. Impression material being injected around the impression post for the recording of an implant level transfer impression.

8. Red acrylic resin being injected onto white impression post for the recording of an occlusal relationship.

9. Digital photo being taken of teeth and shade guide against a black background for the recording and transmission of shade to a technician.

10. Lingual view of telescopic prosthesis on stone model.

11. Mirror view of telescopic restoration.

12. Buccal view of edentulous ridge with a red plastic and a metal temporary abutment.

13. Occlusal view of the extremely narrow edentulous ridge with two temporary abutments.

14. Labial view of implant well and implant sulci after the removal of temporary abutments.

15. Insertion of a laboratory milled non-shouldered abutment into the telescopic crown prosthesis.

16. Prosthesis with seated milled abutment being used to orientate the seating of the abutment into the implant well.

17. Milled molar abutment being seated into prosthesis.

18. Molar abutment seated into prosthesis.

19. Prosthesis being used to orientate the abutment during its insertion into the well of the implant.

20. Telescopic prosthesis being seated on the abutments after their having been tapped into the well of their implant.

21. View of prosthesis immediately after being seated.

22. Radiograph revealing seated prosthesis on non-parallel implants.

23. View of prosthesis ten days after its initial insertion revealing the approximation of the interdental soft tissues.
