Porcelain Fused to Metal: An Aesthetic Treatment of a Metallic Gingival Margin
This case demonstrates the ease with which an unsightly metallic gingival margin on a nine year old porcelain fused to metal restoration can be rectified in a cost effective manner.

     


1. Metallic gingival margin of a nine year old porcelain fused to metal crown on a Bicon abutment.


2. Radiograph of nine year old Bicon implant restorations.


3. Implant well and soft tissue sulcus after the removal of a nine year old Bicon abutment.


4. Laboratory bur is used to prepare the cervical portion of a Bicon abutment by cutting a groove into the surgical grade titanium alloy.


5. After sandblasting with 50 micron alumina and ultrasonically cleaning in 95% ethyl alcohol, the prepared groove is coated with a metallic coupling agent.


6. Porcelain fused to metal restoration with a metallic coated prepared groove is placed in an oven at 120°C for five minutes.


7. A layer of neutral metal opaque powder is applied onto the prepared groove.


8. Restoration with opaque layer is placed into an oven for five minutes at 120°C for five minutes under vacuum.


9. After an application of a ceramo-coupler a second application of an opaque layer is being applied.


10. Restoration is being placed in oven for five minutes at 120°C under vacuum for curing of second opaque layer.


11. After an application and light curing of modeling liquid a polyceramic opaque dentin material is being applied.


12. Opaque dentin is being light cured for 3 minutes.


13. A layer of dentin is being applied.


14. Dentin layer is being light cured for 2 minutes.


15. Superficial layer is being removed prior to polishing.


16. Initial polishing with a super-fine grade carbide bur.


17. Polishing with a pink silicone disc.


18. Further polishing with a nylon bristle brush.


19. Final polishing with a soft bristle brush and a polishing paste.


20. Nine year old porcelain fused to metal crown with an aesthetic gingival margin.


21. Aesthetically improved gingival margin.
