All-Ceramic Crown: Restoration on a Maxillary First Molar
This case demonstrates the use of a relieving incision to allow for the formation of an improved emergence profile with a transitional prosthesis.

1. Occlusal view of palatally positioned temporary abutment.

2. Palatally positioned 5.0mm x 4.0mm shouldered abutment with a poor labial emergence profile.

3. An 8.0mm emergence cuff snapped onto a shouldered abutment demonstrating a poor labial emergence profile. Relieving incision being made to allow for the development of an expanded labial emergence profile by a transitional crown.

4. Relieving incision being made to allow for the development of an expanded labial emergence profile by a transitional crown.

5. One-piece acrylic sleeve being inserted onto the 5.0mm shouldered abutment which is only seated with finger pressure in the well of the implant.

6. Seated one-piece acrylic sleeve.

7. Resin material being injected onto the one-piece acrylic sleeve to fabricate a transitional crown.

8. Resin material being adapted to form transitional crown which will expand the labial emergence profile.

9. Transitional crown incorporating one-piece sleeve which will be shaped and polished to expand the labial emergence profile of the abutment’s sulcus.

10. Initial healing of the expanded labial emergence profile around abutment.

11. View of initial healing sulcus and implant well prior to making an implant level impression.

12. Seated 3.0mm white impression post.

13. Impression material being injected around the white 3.0mm impression post.

14. Implant analog seated on impression post.

15. “Soft Tissue Moulage” by Kerr injected around impression post.

16. Stone model being poured.

17. White 3.0mm implant analog in soft tissue model.

18. Mirror view of all-ceramic crown built on a Bicon aluminum oxide sleeve.

19. Sulcus with expanded labial emergence profile.

20. Metallic coupler cement being applied to abutment.

21. Ceramic coupler cement being applied to aluminum oxide core.

22. Minimal bonding agent being applied to abutment.
