Sinus Lift / Bicon Integrated Abutment Crowns™: Internal Sinus Lift with SynthoGraft™, Two- Stage Placement of a 5.0mm x 6.0mm Short Implant, and Restoration with a Bicon Integrated Abutment Crown™
This case demonstrates the use of SynthoGraft™ pure phase Beta-Tricalcium Phosphate for an internal sinus lift, two-stage surgical placement of a 5.0mm x 6.0mm implant and a 6.5mm x 2.5mm Sinus Lift Temporary Abutment, an implant-level impression, and an insertion of an Integrated Abutment Crown™.

     


1. Pre-operative radiograph.


2. Pre-operative site.


3. Initial incision.


4. Paralleling pin demonstrating the position of the osteotomy after the initial preparation of the osteotomy with a 2.0mm pilot bur.


5. Osteotomy being enlarged with an olive green 2.5mm hand reamer without external water irrigation.


6. Harvested bone within the flute of a red 4.0mm hand reamer.


7. Osteotomy being enlarged with a silver 4.5mm hand reamer without external water irrigation


8. Bicon sinus lift osteotome being used to gently fracture and apically displace a piece of bone.


9. Mixture of patient’s blood and SynthoGraft™ pure phase Beta-Tricalcium Phosphate in a dappen dish prior to being loaded into a syringe.


10. Mixture of patient’s blood and SynthoGraft™ pure phase Beta-Tricalcium Phosphate being injected into the sinus floor.


11. A 5.0mm x 6.0mm implant is being placed with an implant inserter/ retriever.


12. A 6.5mm x 2.5mm Sinus Lift Temporary Abutment being placed into the well of the implant.


13. Implant seating tip attached to a straight handle being tapped to definitively seat the implant fully into the osteotomy.


14. Mixture of patient’s blood and SynthoGraft™ pure phase Beta-Tricalcium Phosphate being placed over the seated titanium abutment.

Second Visit: Five months after the initial visit

18. Incision is being made to uncover the titanium abutment.

Third Visit: Three weeks after the prior visit.



24. White temporary abutment being removed.


25. Integrated Abutment Crown™ being inserted.


26. Floss being used to confirm the interproximal contacts are passive and will not interfere with the engagement of the locking taper.


27. Plastic temporary abutment mounted in an offset handle to seat the Integrated Abutment Crown™ in the long axis of the abutment shaft and implant well to engage the locking taper.


28. Blue markings indicate excessive contact on the occlusal surfaces.


29. Integrated Abutment Crown™ being adjusted with a 7404 carbide bur.


30. Integrated Abutment Crown™ being polished with a pink silicone wheel.


31. Final polishing with a soft goat hair brush and paste.
