Two Full-Arch TRINIA® Telescopic Restorations Featuring Seven Implant Placements and the Use of Both Retentive and Custom Cast Copings for a Seventy-Year-Old Patient in Five Clinical Visits

This case demonstrates the extraction of six teeth and the placement of four maxillary implants (two using sinus lift abutments and others using bone grafting procedures with SynthoGraft™ and Bicon Resorbable Collagen Membranes) and three mandibular implants, uncovering of implants and two conventional implant-level transfer impressions, two occlusal bite registrations, insertion of the mandibular TRINIA® restoration with three cemented Retentive Copings on Universal Abutments and the insertion of the maxillary TRINIA® restoration with four custom cast Sci-tec copings on modified Universal Abutments for a seventy-year-old female patient. The entire treatment was achieved in only five clinical visits during a seven-month period without fabricating any new transitional prostheses.

This treatment demonstrates the extraction of a deciduous maxillary right lateral incisor and its replacement with a 4.5 x 8.0mm Bicon SHORT® Implant adjacent to a congenitally missing canine, as well as bone grafting with SynthoGraft™ in the area of a congenitally missing maxillary left lateral incisor. Guided Surgery with Two-Stage Technique for the Restoration of a 5.0 x 8.0mm Bicon SHORT® Implant for a Mandibular Right First Molar with an Integrated Abutment Crown™ (IAC)

Using the Bicon Guided Surgery Technique, a 5.0 x 8.0mm Bicon SHORT® Implant was placed with a two-stage surgery for a mandibular right first molar and restored with an Integrated Abutment Crown™ (IAC). This technique employs a Spade Drill to initially establish the osteotomy to the diameter of the intended implant. Subsequently, the osteotomy was deepened using incrementally longer Guided Surgery Reamers through the orifice of a 5.0mm Guide Ring until the length of the intended implant was achieved. The top of the Guide Ring was designed to be 15.0mm above the top of the implant when the implant is placed 2.0 to 3.0mm below the alveolar crest of bone.

Replacement of a Failed Screw-Retained Prosthesis with a Telescopic TRIO-TRINIA® Prosthesis with Milled Hybrid Polyceramic Teeth

This treatment for an 83-year-old woman, who presented with a failed maxillary prosthesis supported by five screw-retained IMZ implants, demonstrates the clinical capabilities of a telescopic TRINIA® prosthesis supported by only three Bicon SHORT® Implants: two 4.5 x 6.0-mm implants with a 3.0-mm well, and one 4.0 x 6.0-mm implant with a 2.5-mm well. The laboratory techniques and procedures of this treatment are also presented. After five months of healing, the implants were uncovered and a telescopic TRIO-TRINIA® prosthesis was fabricated with milled hybrid polyceramic teeth.

Laboratory and Clinical Aspects of Fabricating and Inserting Four Universal Abutments for the Immediate Insertion of a Milled PMMA Telescopic Transitional Prosthesis and a Permanent Handcrafted Poly-ceramic Telescopic Prosthesis on a TRINIA® Framework with Three Milled Copings and One Custom Cast Chrome Cobalt Coping with an Interchangeable Retentive Attachment

This treatment depicts the detailed aspects and nuances of the above-described prostheses in only three clinical restorative visits, which included:
During the first visit, the surgical uncovering of four implants, an implant-level transfer impression, the recording of an initial occlusal relationship, and choosing an aesthetic tooth shade.
During the second visit, the extraction of the four teeth, insertion of three unmodified and one modified Universal Abutment, which was unwisely modified to achieve an unnecessary aesthetic effect by using a custom cast chrome cobalt retentive coping, a second occlusal registration, and the insertion of a milled PMMA telescopic full arch transitional prosthesis, with gingiva-colored Ceramage material added for aesthetics, to confirm the appropriateness of the teeth arrangement as well as a transitional device for function while the extraction sites were healing and the permanent TRINIA® restoration was being fabricated. Alternatively, as a less expensive option, the PMMA prosthesis could have been affixed to the abutments without the telescopic copings.
During the third visit, the insertion of a handcrafted poly-ceramic telescopic restoration on a TRINIA® framework with three cemented milled retentive copings and one custom cast chrome cobalt coping with a yellow SiTec retentive attachment, as well as the intra-oral relining of the prosthesis with Ceramage.

Guided Bone Regeneration Technique and Implant Placement in an Atrophic Mandibular First Molar Site

This treatment offers an alternative to ridge splitting procedures, demonstrating the molar implant placement and simultaneous guided bone regeneration of a 56-year-old woman’s atrophic left mandible. Because of the very thin and dense crestal bone, a high-speed drill was used to initiate the osteotomy prior to the successive use of latch and hand reamers, and the placement of a 4.5 x 6.0mm implant, which was covered with SynthoGraft™ within a collagen membrane pocket. The 5-year post-insertion clinical and radiographic images reveal a successful treatment with significant bone gain.
