サージカルガイド使用・インターナルサイナスリフト法、両術式を同時に応用し、5.0×6.0㎜のインプラント埋入、最終クラウンをインテグレーテッドアバットメントクラウンとした臨床例を紹介する

この症例ではソケットリフト(インターナルサイナスリフト)含む2回法の術式を紹介する。上顎左の第一大臼歯部位に5.0×6.0㎜のインプラントを埋入した。インプラントシミュレーションソフト上での治療計画により、サージカルガイドをデザイン、作成した。5.0×5.0㎜のインプラントを埋入する予定のサージカルステントを作成し、上顎洞底への侵襲を極力少なくなるように計画した。インターナルサイナスリフト法をして、5.0×6.0㎜のインプラントを埋入することにした。オステオトミーは5.0㎜径のインプラント埋入用のサージカルキットを使用。6.0㎜長径まで順序よくリーミングしていった。最終クラウンセット後、1週間、9か月、5年後のレントゲン写真も紹介している。

In diesem Fallstudienvideo wird die zweizeitige Platzierung eines 5,0 x 6,0 mm Bicon SHORT®-Implantats mit einem internen Sinuslift unter Verwendung einer “guided surgery” für einen rechten Oberkiefermolaren und dessen Wiederherstellung mit einer “Integrated Abutment Crown ™ (IAC)” demonstriert. Die Platzierung der geführten Operation wurde für ein 5,0 x 5,0 mm kurzes Implantat konzipiert, um das Risiko einer Sinusperforation zu minimieren und die Platzierung des 5,0 x 6,0 mm Bicon SHORT®-Implantats mit einem internen Sinuslifting zu beschleunigen. Die Osteotomie wurde dann unter Verwendung der “guided surgery Reamer” den 5,0 mm-Guidet Ring vertieft, bis die Länge der beabsichtigten Osteotomie und Sinuselevation für das 6,0 mm-Implantat erreicht war. Darüber hinaus werden Follow-up-Bilder nach einer Woche, neun Monaten und fünf Jahren bereitgestellt.

Video nghiên cứu này trình bày việc cấy ghép hai giai đoạn một Implant Bicon SHORT® 5.0 x 6.0mm với quy trình nâng xoang nội, sử dụng phẫu thuật có dẫn hướng cho răng cối đầu tiên của hàm trên, và phục hồi bằng mão răng tích hợp abutment (IAC). Phẫu thuật cấy ghép có dẫn hướng được tiến hành cho một implant 5.0 x 5.0mm, nhằm giảm thiểu nguy cơ thủng xoang và đẩy nhanh việc đặt Implant 5.0 x 6.0mm Bicon SHORT® với quy trình nâng xoang nội. Lỗ khoan sau đó được làm sâu hơn bằng cách sử dụng Mũi khoan phẫu thuật có dân hướng dài hơn thông qua Vòng hướng dẫn 5.0mm cho đến khi đạt được độ sâu cần thiết để tiến hành nâng xoang dự định cho implant 6.0mm. Ngoài ra, video còn có hình ảnh theo dõi sau một tuần, 9 tháng và 5 năm.

Двухэтапный протокол установки по шаблону имплантата Bicon 5,0 х 6,0 мм в область первого правого моляра ВЧ с закрытым синуслифтингом и его протезирование интегрированной коронкой

В клиническом видео показан двухэтапный протокол установки по шаблону короткого имплантата Bicon 5,0 x 6,0 мм в область правого первого моляра на ВЧ. Одномоментно проведена операция закрытого синуслифтинга. В последующем установлена интегрированная коронка (IAC). Ложе под имплантат формировали через хирургический шаблон, смоделированный под имплант размером 5,0 х 5,0 мм, чтобы минимизировать риск перфорации пазухи и с максимальным контролем провести закрытый синуслифтинг при установке короткого имплантата Bicon 5,0 х 6,0 мм. Остеотомию постепенно углубляли с помощью более длинной фрезы для имплантации по шаблону через направляющее кольцо диаметром 5,0 мм до достижения нужной глубины и поднятия дня пазухи на высоту, необходимую для установки имплантата длиной 6,0 мм. Видео дополнено контрольными снимками через 1 неделю, 9 месяцев и 5 лет после имплантации.

Two-Stage Placement of a 5.0 x 6.0mm Bicon SHORT® Implant with an Internal Sinus Lift using Guided Surgery for a Maxillary Right First Molar and its Restoration with an IAC

This case study video demonstrates the two-stage placement of a 5.0 x 6.0mm Bicon SHORT® Implant with an internal sinus lift procedure using guided surgery for a maxillary right first molar and its restoration with an Integrated Abutment Crown™ (IAC). The guided surgery placement was designed for a 5.0 x 5.0mm implant in order to minimize the risk of sinus perforation and expedite the placement of the 5.0 x 6.0mm Bicon SHORT® Implant with an internal sinus lift procedure. The osteotomy was then deepened using incrementally longer Guided Surgery Reamers through the 5.0mm Guide Ring until the length of the intended osteotomy and sinus elevation was achieved for the 6.0mm implant. In addition, one-week, nine-month, and five-year follow-up images are provided.
